Lack of health literacy, lack of transportation services, and food insecurities among the socially vulnerable population are some of the primary reasons for healthcare overutilization. Using data science and augmented analytics, we are determining the socially vulnerable population and predicting readmissions. This will help healthcare companies reduce their costs related to readmissions, unnecessary visits to ED, and prevent health conditions from becoming chronic.
